  • Fraternal Order of Eagles No. 624, 1104 S. Oak St.: Nov. 9: One violation: Sanitizer not used according to label instructions.

  • Rose Bowl, 1600 N. Central Ave.: Nov. 2: One violation: No approved food manager’s certificate posted in establishment.

  • The Spot, 500 N. Central Ave.: Nov. 9: Three violations: Handwashing sink holding dishes and unavailable for handwashing; ice scoop stored with handle in ice; lights in kitchen not shatter resistant or shielded.

Nekoosa

  • Goodfella’s Pizza Pub, 225 First St.,: Oct. 28: Five violations: Employee plated food with bare hands; sanitizer not used according to directions (corrected during inspection); handwashing sink not readily accessible for proper handwashing; cutting board near salad area scored; wall to right of salad preparation cooler soiled.

  • United Church of Christ, 205 Market St.: Nov. 22: No violations.

Pittsville

  • Dupe’s Diner, 5295 Second Ave.: Nov. 18: No violations.

  • Little Bull Falls, 6087 Wood County A: Nov. 16: One violation: Milk in cooler exceeded date mark.

Wisconsin Rapids

  • America’s Best Value Inn, 3410 Eighth St. S.: Nov. 30: No violations.

  • Hoozels, 4010 Plover Road: Nov. 4: No violations.

  • Shaw Lee Restaurant, 222 W. Grand Ave.: Nov. 10: Two violations: Rice spoon stored improperly; racks in reach-in cooler rusty.
